LIABILITY WAIVER AND RELEASE FORM (adults)
I, the undersigned, hereby affirm that I am in good physical health and fully capable of participating in the Grapplix Jiu-Jitsu competition. I acknowledge the inherent risks associated with my participation, including but not limited to physical injury or even death. By signing below, I voluntarily agree to assume all responsibility for any risks, injuries, losses, or property damage I may sustain, including death, during my participation in this event.
I fully understand that the physical activities involved may pose a hazard to me, my property, and others, and I willingly assume the risk of any potential harm that may occur. I further acknowledge that this event will be broadcast on live Pay-Per-View (PPV), and I consent to the possibility of my likeness being captured and distributed through this medium.
I certify that I am at least 18 years of age.
In consideration of being allowed to participate in the Grapplix Jiu-Jitsu competition, I, on behalf of myself, my heirs, assigns, and personal representatives, hereby RELEASE, WAIVE, DISCHARGE, AND COVENANT NOT TO SUE Grapplix, its officers, directors, employees, agents, volunteers, sponsors, and any other associated parties (hereinafter referred to as the Releasees) from any and all liability, claims, demands, or causes of action, including but not limited to injury, death, or property damage arising out of or in connection with my participation in this event or while on the premises where the event is being conducted.
This release and waiver of liability shall bind my family members, spouse (if applicable), and my heirs and personal representatives in the event of my death. This agreement shall be governed by and construed in accordance with the laws of the United Kingdom.
By submitting this form, I consent to being contacted regarding future events, promotions, and updates from Grapplix. I acknowledge that I have read and fully understand this Waiver of Liability and Hold Harmless Agreement. I am signing voluntarily and of my own free will, without any inducement or oral representation apart from the terms outlined herein. I execute this release with full knowledge of its legal implications, fully intending to be bound by its terms.

I acknowledge that Jiu-Jitsu is a physically demanding sport that carries inherent risks, including but not limited to serious injury, disability, or death.
By signing this waiver, I voluntarily accept full responsibility for all risks, injuries, losses, or damages—including death or property loss—that I may sustain as a result of participating in this competition or related activities.
Media Consent
I understand that this competition may be broadcast via Pay-Per-View (PPV) or other media platforms, and I hereby grant Grapplix the right to use my name, likeness, image, voice, and appearance in connection with any media coverage, promotional materials, or broadcasts—without compensation.
Waiver and Release
In consideration for being permitted to participate, I, on behalf of myself, my heirs, assigns, and personal representatives, hereby:

Grapplix, its officers, directors, staff, referees, volunteers, agents, sponsors, venue operators, and all other associated parties (hereinafter referred to as the Releasees) from any and all liability, claims, demands, or causes of action, including but not limited to injury, death, or property damage, arising out of or in connection with my participation in this competition or while on the premises where the event is conducted.
Binding Agreement
